Authority to proceed

I/We have read and understood this Statement of Advice (SoA) and acknowledge the following:

1. I provide Cole Murray with authority to proceed in implementing the advice contained within the Statement of Advice.
2. All investments carry risks, and the return I expect may be different from what is stated. I understand that there is no guarantee of getting back some or all of the capital and interest when needed.
3. That all the information that I have supplied is true and correct and nothing that I deem ‘material’ has been omitted.
4. That my circumstances could change over time, and it is my responsibility to contact my adviser when changes occur that could alter the applicability of this Statement of Advice.
5. That all funds to be invested will be done so from funds gained by lawful means.
6. If the scope of advice relating to this Statement of Advice is Limited Advice, it has been limited by me and not by the adviser. I acknowledge the advantages of receiving a Full Financial Plan rather than a ‘limited service’.
7. I have the right to access and to correct my personal information that is held on file.
8. Permission is granted for Cole Murray Group advisers, staff, and NZ regulatory bodies to share or review my information as needed to implement recommendations, verify identity, and ensure regulatory compliance.
9. I will provide the adviser with any change of address (including email) plus any changes in my Prescribed Investor Rate (PIR).
10. That the relevant Disclosure Statements have been provided by the adviser and I have read and understood them.
